Breast Augmentation Options

Brest augmentation is the most common plastic surgery procedure among women today in the United States. Over 300,000 women per year undergo this procedure. The dominant method used to be through silicone implants until 12 years ago, where this was stopped as a general procedure. Now over 90% of breast implants used are saline implants. The issue is one of leaks. If saline solution leaks out of an implant it is just absorbed by the woman's body, while silicone leaking can cause problems in surrounding tissue.

These problems are not as severe as were thought a decade a go. Now, about 10-percent of women have silicon implants used, as careful test study. The FDA still has to make a ruling on the general use of silicon implants. The main problem is silicon leaks can create a higher rate of capsular contractures, which cause lumps in the breasts, and require additional surgery to fix, but these problems can occur with saline breast implants as well.

There are other issues for you and your surgeon to decide on when implanting breast implants. The implant can be put either above or below the pectoralis muscle in the breast. The advantage of an implant below is they do not disturb having a mammography, and have less risk of capsular contractures (lumps). However, this is a more complicated surgical procedure. If the implant is put on top of the muscle, but under the gland, it will make it difficult to have a good mamograph, and the increase danger of capsular contraction. For some women, putting the implant above the muscle creates better projection, and less drooping of the breast. Also different shapes of implants are available, some of which project more than others. Breast implants can last for a long time, but they may have to be checked often and eventually replaced.

Problems include capsular contraction, which is the formation of scar tissue around the implant to form a hard shell, sort of a large lump. Over time there is a danger that the implant itself will shift its location and require additional surgery to correct it. Also there will be some scars, even though present techniques tend to minimize them. There will be much more noticeable scars if along with a breast augmentation, a breast lift is performed. This is an actual moving, or "lifting" of the location of the breast's nipple. Breast sensation will decrease often, but return in one to two years. Fifty percent of women who have this surgery cannot breast-feed infants.
